Its not a fantasy...its in the begining and before she strated sleeping. It did really happen and i think that the old couple are two strangers for Betty (who met them during her flight) but the parents (or grandparents) of Dianne. People generally tend to be accountable for their actions first to their familly members. I think thats the reason why the old couple "terrorizes" her at the end, representing her guilt and the shame for her action to kill her ex-girlfriend.
